At
The Village at Alameda we recognize that each resident is an individual
with individual needs. That is why we meet with the resident and family
to conduct an assessment prior to move-in. These assessments are
reviewed every six months or with any change in condition to ensure
that we meet the resident's ongoing and changing needs. We develop an
individual service plan for each resident
Our Nursing Services Include: (but not limited to):
- Licensed Staff to Provide Sliding Scale Insulin Shots
- Medication Assistance (including coordination between physician & pharmacy)
- Assistance Ambulating
- Assistance with Transfers
- Assistance with Personal Activities such as:
- Dressing
- Personal Hygiene (Oral/Denture Care, Shaving, etc.)
- Showering
- Toileting/Incontinence Program
- Select Diabetic Services
- Co-ordination with Healthcare Providers
- Reminders and/or Escorts to Meals and Activities
- Licensed Nurses on Staff 7 Days a Week and On Call for After Hour Emergencies
Optional Services that We Offer for Your Convenience:
Physician “House Calls” made by independent physicians for our more fragile residents that may not be able to go to appointments
